Confusion and speech disconnection in a schizoaffective patient.?

I go through periods of time where my speech is disconnected and i can't understand whats going on around me. I already hear voices and have kinda sorta been diagnosed with schizoaffective disorder, what could possibly be going on now? (I'm only 21)
